Please complete this form if you are requesting the costs associated with ESYA sports to be covered by the ESYA Scholarship donation.
Complete a new form at the start of each ESYA sport season for each child you have participating in that ESYA sport. Your information will be kept confidential and will only be viewed by the ESYA President, Vice President, and Treasurer.
